The Job

We are looking for an experienced qualified Nurses to join our team and to undertake the duties and responsibilities listed below:

Provide clinical evidenced based interventions, as detailed in the Care Plan and determined by the Care Coordinator, to service users with complex and substantial needs, under the direct supervision of the Pathway Clinical Lead or Care Coordinator.
Offer clinical advice and information to service users, carers, other professional.
disciplines, voluntary organisations, and the public.
To engage in active relapse prevention planning with service users and carers,
contributing to their recovery and discharge planning.
Contribute to the service user’s transition across all services provided by the Compkey Healthcare and adhere to standard work procedures.
Administer medications (oral and intramuscular) when required and will also monitor for therapeutic responses to medications and potential side effects in a variety of environments, e. g. service user’s home, and liaise with the prescriber. (Consultant Psychiatrist or GP as appropriate) to ensure optimum outcome for service users.
Recognise and anticipate situations that may be detrimental to the health and wellbeing of service users and their Carers and advise and treat on the promotion of health and the prevention of physical/mental and behavioral deterioration.
Support service users to contribute to their own individualised Care Plans, and Wellness Recovery Action Plans (WRAP) / other Recovery Plans in relation to their Recovery and discharge planning.
Use Digital Recording Equipment and mobile technology as per Compkey healthcare Policy.

Requirements:

At least 1-3 years of experience as a Nurse.
A degree level qualification in healthcare.
Evidence of up to date: Professional Nursing qualification e.g., MN/RNMH/ RNLD • Continued professional development • Experience of working with service users with complex needs.
